 An After School Activities Form must be completed before a student can stay for any activity and can be picked up in the main office.

**Please note: Art club will rotate 10 students every 9-weeks. If your child would like to participate, please have them write 5 sentences explaining why they would like to be a part of Art Club for a 9-week period and attach it to this permission slip. Please note: once your child participates in Art Club, they will not be permitted to participate in a second rotation of Art Club.
